This form is used to request services from AccessMU and should only be completed by faculty and staff.

End of Life OS Exception Request (legacy)

This form is used to request an exception for an EOL OS machine.

IT Services Secure Disposal (legacy)

Members of the IT Business & Administrative Services team (BAS) may use this form to submit requests for the secure disposal of IT Services owned desktops, laptops, and tablets.
