FBS Divisional Help from FBS-IT

Introduction

This form applies to ALL departments in the Finance and Business Services Division.  Staff in those offices will need to use this form to request help from FBS-IT staff for a variety of technical issues or tasks related to Banner and Bannerweb for the HR/Payroll Module), Banner Document Management (BDM/Imaging), ​​BI (OBIEE), FM systems (PFD) or other systems/processes being used by those offices not otherwise listed. 

This form consolidates information and streamlines the previous method of asking for assistance via general email to any of the following: Dan Murphy, Jen Herman, Chris Weitzel, Patricia Makaroff, Angeline Poling, Paul Benedetto, George Salem, Kirk Hopkins or Sarah Persinger (when no other ticket type is applicable to use).

This request form is not to be used for desktop computer or printer support (see below for the link to the request form to use for those purposes) or for staff outside of the FBS division.

Requests for assistance using this form are typically fulfilled within 4 business days (though there is an option to indicate whether the request has an urgent need for a faster response).
