Specific Service Request forms that you might find helpful:

- Technology Support Services: Purchase and/or Deploy a University-Owned Computer (this routes to Technical Support Services (TSS) staff, the group responsible for your desktop, laptop and printer support)

- Desk Phone, Call Center, Fax support requests (including XMediusFax service) - this routes to the appropriate Central IT team for assistance

This help request form applies to ALL departments in the Finance and Business Services Division. Staff in those offices will need to use this form to request help from FBS-IT staff for a variety of technical issues or tasks that are not in the list above. These tasks could be related to Banner and Bannerweb (for the Finance and the HR/Payroll modules), Banner Document Management (BDM/Imaging), BI (OBIEE and OAS), FM systems and TMA (used by PFD), Jaggaer (Buyway) or other systems/processes being used by our divisional offices but not otherwise specifically listed here.
This form consolidates information and streamlines the previous method of asking for assistance via general email to a specific individual.
This request form is not to be used for desktop computer, printer, desk phone, or fax support (see above for the link to the request forms to use for those purposes) or for staff outside of the FBS division. This is also not to be used for file permissions updates - the link for that is noted above.
Requests for FBS-IT assistance using this form are typically fulfilled within 4 business days (though there is an option to indicate whether the request has an urgent need for a faster response).
