Introduction
Members of the IT Business & Administrative Services team (BAS) may use this form to submit requests for the secure disposal of IT Services owned desktops, laptops, and tablets. A member of TSS will pick the device up from the location specified, securely wipe it, and then either recycle the device or turn it over to the F5 program.
