2. Provide the following information:
   • Reason for access
   • Name of the contractor or employee who needs access
     • Contact information
   • Name of contractor company (if applicable)
   • Location of communication room(s), including building name and room number
   • Miami Project Manager
3. Refer to the Support Guide: Telecommunication / Room access request
