This form should be used to submit captioning and audio description requests for Videos and Live University Hosted Events. This form should NOT be used to submit requests for students who are affiliated with Student Disability Services and are eligible for captioning/interpreting accommodation. If you need to submit a request for students with captioning accommodations, please do so using Students Accessing Miami (SAM).
Billing:
Individuals submitting captioning, interpreting, or audio description services will be responsible for the expense of those services.
