Objective
To provide instruction on obtaining a FileVault recovery key to unlock an encrypted disk when:
You are locked out after restarting your system — it is asking you for a FileVault recovery key
Environment

Resolution
To request a recovery key,
complete and submit the form for requests requiring photo identification
Under
Password Reset Request Type
, check the box to select
Other
Provide the following reason for making your request:
Locked out of encrypted disk
Contact your local desktop support representative
Local desktop support will, in most cases, have access to recovery keys and may be contacted for assistance, if you are unable to access a locked disk
