Request a recovery key for macOS / FileVault

Problem
- How do I obtain a FileVault recovery key to unlock an encrypted disk when I have been locked out of the system
- I was locked out after restarting my system — it is asking me for a FileVault recovery key
Resolution
- To request a recovery key, complete and submit the form for requests requiring photo identification
- Under Password Reset Request Type, check the box to select Other
- Provide the following reason for making your request: Locked out of encrypted disk
Contact your local desktop support representative
- Local desktop support will, in most cases, have access to recovery keys and may be contacted for assistance if you are unable to access a locked disk
