Formstack / Questions

Objective

• To provide direction for requesting a Miami Formstack account
• To provide access to up-to-date end-user instructions and support documentation for Formstack
• To provide links to the comprehensive Formstack support site and status dashboard

Rationale

• IMPORTANT NOTE: Miami will sunset our use of Formstack in the summer of 2025. We encourage you to use the other form tools available instead of creating new forms in this system so that you won’t have to create them a second time. If you already have a Formstack account, begin planning where your active forms can move to and work on a transition plan for your office. No new department/office accounts will be created but new users can be added to existing office accounts while we transition away from this tool
• Formstack is a commercial website that allows you to create online forms using drag and drop functionality within a Miami brand template. A signed contract with the vendor guarantees its accessibility
  • Use it in place of paper forms and applications; to collect information or conduct surveys
• Students are not permitted to have access to Formstack. Do not allow a student to access Formstack under another user's account.

Resolution

• Request a Formstack account 
  • If requesting access to create and edit forms:
    • After you request an account, you are required to complete online Formstack Training and submit the Formstack User Agreement within seven days of receiving account access. The online training course takes about 30 minutes
  • If requesting access to view submissions or approve forms only:
    • After you request an account, you are NOT required to complete online Formstack Training. You will be given access immediately.
