Resolution

  1. Complete and submit your request for repair of your cable outlet 
  2. Provide the following information:
    • Nature of your request
    • The location of the data jack, including building name and room number
    • The jack ID listed on the sticker above the jack
    • Your location, including building name and room number
    • Your contact information
