Please read these important details before submitting your request!
• We will notify you when the account has been created to allow you to notify the contractor/supplier download their digital ID to their smartphone or stop by the Campus Services Center located in Nellie Craig Walker Hall, room 034 to have a physical ID card printed. Please do not send them to our office until we have notified you by email that the account has been created.
• Before the digital ID can be downloaded to their smartphone, they must have an approved photo on file. Please attach the contractors ID photo to this request.
• Details about the digital ID and downloading information are available at:
https://miamioh.edu/campus-services/home/miami-ids/digital-id/index.html• The newly created PIK (account identifier) will also be provided to allow you to work with your Building Point of Contact (BPOC) to add door access to the account.
• Instruct the contractor/supplier to bring a photo ID with them for verification purposes when we take their photo and print the card (ie. driver's license).
• The first contractor/supplier ID card is provided at no charge. Lost ID card replacement cost is $35.00 and must be paid for in the office via credit/debit card. A broken/damaged card will be replaced at no charge.
• The broken card must be turned in when a replacement is printed. Failure to turn in broken/damaged card will result in the lost card charge of $35.00.
