Rationale

• Miami provides the Qualtrics survey tool for use at no cost to faculty, staff, and students. Users can create and distribute their own surveys and gather information in support of the University's educational mission and organizational goals

Resolution

1. Determine whether your survey needs approval from Miami University's Instructional Review Board for Human Subjects Research (IRB)
   • For more information, go to the Institutional Review Board web site
2. Go to the Miami Survey Tools site and click the Log in to Qualtrics link
3. If prompted, log in using your Miami UniqueID and MUnet password
4. Go to Qualtrics Support for instructions on creating a survey project

• If you do not currently have a Miami account for Qualtrics, one will be generated for you automatically the first time you log in
• Qualtrics has been reviewed by Miami's security team and approved for conducting surveys at Miami
